YOU MUST COMPLETE THIS CANCELATION FORM using the NAME and EMAIL you registered with to cancel your membership. Our team will process your cancelation and remove you from the members-only Facebook Group within 24 hours.
Please keep in mind, all cancelations must occur 48 hours before your renewal in order to give our office time to process your cancelation. (For example: If you joined on the 5th of June, then you would need to cancel by July 3rd in order to avoid another billing cycle.)
